i dont understand which i should answer on the accelerated procedure AST form
should i tick answer 4 or 5

what does that mean on 5C " on the .......... (enter date here) (before tenancy began) a notice in writing, stating that the tenancy was a AST, was served on the defendants. IT was served by......

and 5d says attach to this claim form a copy of that notice

I gave a section 21 notice to the tenants dated 13.01.13
tenants moved in on 29.09.12
said on notice will start proceedings after 29.03.13

Took a photo and put the notice on the tenant door. As have been told by police to stay away from here.
Have photo evidence but shows date on camera as taken on 15.01.13 - I am not sure whether to now add the date on the notice or the date that shows on my camera. There is a difference of 2 days.

question 7 is asking for proof of service - er how am i going to do that as i stupidly didnt think it through so cant print my date on my camera
