I'm new to this.

I will rent the house to 2 professional working adults who will pay the rent.
2 of their elderly parents will live there too. They won't be contributing to the rent.

Do I put them all (all 4 adults) on the tenancy?
Do I need to do any vetting/screening of the older couple?

I'd insist on them all on the agreement as tenants with all the normal checks and vetting.

Absolutely. If you have other suitable applicants I would give this a miss.
