Hi, ive rented my house to tennant since 18 Dec 2010 every 12 months this is when she moved in, and i have issued a tenancy aggreement every year since then. back in April 2015 her boyfriend left, and i issued a new tennacy agreement on the 1st May 2015 for 12 months , and the last tenancy agreement issued was on 1st May 2016 for 12 months and i haven't renewed the tenancy agreement so i believe it automatically goes to a rolling month to month periodic.
Anyway i just want back possession of my house can i issue section 21 form6a  downloaded from the dept of gov,  the form has section 21(1) and (4) is this the right form for me?
And when filling the names out do i put exactly how its written on the tenancy agreement including my name? I never put my full name just intial and surname with Mr,  do i include Mr in the name of signatory?  Hope someone can help
