Nightmare agent - Non paying tenant - HMO

We have had an HMO for about 9 years now in Bristol. We are having issues with one of the tenants and are also planning on selling the property soon. Unfortunately, the Agent we used has made a complete mess of it. Tenant started in August on a 6 month AST.

The issues:
•   Standard tenancy agreement used from RLA instead of room only tenancy agreement
•   No "How to rent" guide, EPC & copy of gas certificate given on start of tenancy
•   Deposit not protected within 30 days
•   Tenancy agreement filled in incorrectly: The tenants address for notices is under the landlord's address for notices, no address for landlord on tenancy. May have been given separately?
•   Tenant has so far only part paid the first month rent (2 months late) so is now almost 3 months overdue.

What we have done so far to rectify the problems:
•   Issued section 8 notice on tenant at HMO address
•   Protected deposit with DPS & issued prescribed information
•   Send out section 48 notice to rectify landlord notice address

Questions:
1.   Should we issue another section 8 notice to the address on the tenancy agreement even though as it stands this was the address to be used for the landlord? Or could we pursue already issued section 8 notice in court?
2.   Should we provide the tenant with the "How to rent" guide & copy of gas certificate now?
3.   Am I correct in thinking an EPC is not needed for HMO's? (We pay all the bills apart from phone/internet)
4.   Anything else I missed or any other suggestions on how to proceed?
