N5b form - name, date and service queries

Hoping someone can help.

Just filling in a n5b form and could do with clarification on some points if thats ok.

My husband and I are joint landlords, however I use a letting agent to manage the property, which is based in Wales. It is a standard semi detached with a standard ast (only one raised). The tenancy commenced on 5 December 2015 and because its based in Wales I raised a s21 (not 6A) in our joint names. The s21 was raised on 31/1/2017 and expired after 4/4/2017. The letting agent posted it first class on 31/1/2017 therefore I believe the date of service to be 2/2/2107. I have no proof of postage as the letting sent using thier franking system so the letting agent has filled in a n215 certificate.

1) First page - In the ast it states 'Landlord(s): Landlord' Agent: xxxx Lettings, xxx Street, Wales, so has made no mention of our names. However the prescribed inf that is attached to the ast states both our names however under the section landlords, with c/o xxx Lettings. As I am raising the n5b myself I have put the claimants as myself and my husband c/o XX Lettings and their address - is this OK?

2)Point 2 -I have also noticed that the ast is dated 2 December 2015 and the tenancy started on 5 December 2015, which date should I put in point 2?

2)Point 4 - I have used the online process to fill in the n5b. It has not lined through point 4 - do I need to do this?I have just ticked n/a to parts a and b

3) Point 5 - I have simply put it was raised by 'claimants and sent first class post by xx Lettings.Is that OK?

4) Point 5 evidence C1 - will the n215 filled in by the letting agent be adequate enough proof of service?

5) Point 7 (i) and (iv) - The deposit is lodged with the DPS by the letting agent - I have asked the letting agent to send me the certificate. It states Tenancy Deposit Certificate. I will redact the repayment ID before attaching. However is (i) and (iv) the same?
