Can anyone write up a Guarantor agreement and sign it?

Can a Guarantor form be only 5 lines and not mention the address or the landlord or the terms or the actual amount of rent?

Reason I'm asking is I believe my tenant wrote a letter and signed it on behalf of a would be guarantor and it was overlooked at the time because rent was actually being paid on time for about 30 months in a row - but guess what?  The rent stopped and the tenant is still there and telling me to chase the guarantor. The guarantor has asked me who I am.

I think I've lost my rent...is there any way I can realise any of the arrears...or was I far too kind to my tenant?

No you have been stitched up a treat.
Evict the tenant.
You would need a deed attached to the AST and even then you would need to go through the useless County Court processs.
Use RGI next time.
